Patient Care Coordinator Role
(part-time with full-time opportunity)

At Moment, you will never feel like you’re being put through a factory. We know that the industry standard is one physical therapist managing anywhere from 2-8 patients within the same hour. That means that the patient is probably not getting the attention to detail that they need and the physical therapist serving them is burnt out. No one wins in that model which is why we are changing the way people experience physical therapy.

About the Role:

As a Patient Care Coordinator, you will serve as the main point of contact for patients and the physical therapists on our team. You will use your understanding of our customer journey, backend systems, standard operating procedures, and more to make sure that there is a seamless experience regarding anything that is not clinically related. You will make sure that the patients have their intakes filled out before evaluations, scheduling is inputted correctly, invoices are created for sessions, payments are processed, patients are being followed up with, and more.

What You'll Do:

  • Manage inbound phone calls and emails

  • Send patient intake forms before evaluations

  • Schedule patients into the EMR software

  • Create invoices for patient appointments

  • Process payments after patient appointments

  • Send out superbills after patient SOAP notes are completed

  • Collaborate with physical therapists to ensure completion of the plan of care

  • Follow-up with current patients who have fallen off on their plan of care

  • Follow-up with previous patients who have completed their plan of care

  • Be the point person to answer questions related to back-end operations

  • Work closely with the team to ensure data entry quality and consistency

What We're Looking For:

  • 1-2 years experience in health care administrative role (preferred)

  • Independent self-starter and adaptable in a fast-paced and changing environment

  • Excellent communication skills (written and verbal)

  • High level of attention to detail

  • Ability to work under pressure and strict deadlines; flexible

  • Ability to work well with ambiguity, solve problems, and think on your feet

  • Ability to empathize and communicate with patients

  • Willing to learn, be receptive to feedback, and want to continuously improve

  • Ability to identify larger issues and make recommendations for improvement

  • Resilient capacity to deal with pressure and the demands of the workplace (remote/office)

  • Bachelor's (BA/BS) degree, specialization in natural sciences, healthcare, administration, or hospitality is a plus

We are flexible with our role meaning it can be in-person, virtual, and/or hybrid. Our goal is to begin part-time and grow the role into a full-time position as tasks and duties evolve.
